Preparing for your first visit with a new dentist can be overwhelming, especially when they aren’t clear about what you can expect and will need to bring to your appointment. We want to make that process as stress-free and simple for you as possible. Below, we’ve included some helpful information, including our new dental patient forms and an application for financing, if needed. Your First Visit

After you give our office a call to schedule your appointment, we’ll have you either fill out your new patient forms in the waiting room when you arrive, or you can do it online by accessing the links in the “Patient Forms” section below. Your first visit with us will be with Dr. Malley or Dr. Abernathy—they’ll complete your cleaning as well as a thorough examination of your teeth and gums. We’ll inform you of any findings and create a treatment plan to get them addressed, being sure to schedule any necessary appointments before you leave. After this initial visit, all of your following hygiene appointments will be with our talented and highly experienced dental hygienists, Debbie or Monica.

For your convenience, you can fill out our new patient forms in three different ways: at our office, once you arrive, by downloading them and filling them out at home, being sure to bring them in with you, or online. Either way, we ask that you please do this ahead of time to help streamline your visit and ensure our team has all of the information we need to get you started.

Our office is in-network with AlwaysCare, and we’re happy to file claims for any PPO dental insurance plans to help patients who aren’t in-network receive a maximized reimbursement. If you have any questions about your insurance coverage, our experienced front desk staff has been filing claims for years, so they’ll have all of the answers you need to understand your plan better.
